Follow the Child Program

City-County Health Department and Department of Family Services have teamed up to implement a new program called Follow the Child.  A public health nurse from the Health Department will be working with children age 0-18 years of age in foster care in Cascade County. 

The nurse will provide health care case management, ensuring that each child receives all scheduled and recommended medical care, including: visits to primary doctors, immunizations, dental exams and treatment, eye exams, developmental screening and follow-up, mental health care, physical and speech therapies, and other care as needed.  Home visits to the children and their parents/foster parents will assess the needs of the children based on past medical care. The nurse will then coordinate any referral necessary for complete medical, dental or emotional care. Clearly defined medical goals will help DFS to assist with completion of the plan. The nurse will compile a health history summary which will follow the child throughout care, and which will be turned over to the child when he or she reaches adulthood.
