Community Transformation Grant Visual Preference Survey

This survey is a result of the Community Transformation Grant. Community transformation is about helping our communities to be healthier by making the healthy choice the easy choice. An important part of this work is to make the public aware of different options for how we build and maintain our roads, sidewalks, parks, and other infrastructure elements in our communities. Most often this work happends through the vision provided in your local Growth Policy, Transportation Plans, and other documents.

Getting community members involved in providing input into these policies and plans is really important. It helps planning departments to understand the preference of community members. However, it is difficult to get a large number of people to attend a public meeting!

This survey will allow you to tell us how you would like your community to look and what is important to you without leaving your home or office. You will be shown a few pictures of what is possible and asked to tell us what you like.

We hope you have fun and enjoy taking this survey. If you have any questions about the survey, please feel free to call Alicia M. Thompson, MSW, Health Officer:Cascade County, President: Montana Public Health Association, at 791-9260 or email
